Regulamin Black Swan Pracownia Tatuażu
Klient, który decyduje się na zabieg wykonania tatuażu jest zobowiązany do zapoznania się z treścią regulaminu, przez co akceptuje jego warunki.
• Zabieg wykonuje osobom pełnoletnim. Osoba, która nie ukończyła 18 roku życia może zostać poddana zabiegowi tylko przy obecności opiekuna prawnego (tylko powyżej 16 roku życia).
• Każdy klient (lub w przypadku osób niepełnoletnich opiekun prawny) jest zobowiązany do podpisania formularza potwierdzającego zgodę na jego wykonanie oraz formularza zdrowotnego.
• Osoba tatuowana nie może znajdować się od wpływem alkoholu, narkotyków oraz innych środków odurzających, a o wszelkich przyjmowanych lekach należy obowiązkowo poinformować.
• W razie jakichkolwiek komplikacji lub wątpliwości odnośnie gojenia się tatuażu, należy kontaktować się osobiście.
• Nie ponoszę odpowiedzialności za sposób zachowania się barwników po zabiegu, nie odpowiadam za powikłania będące skutkiem zabiegu.
• Po zabiegu zawsze informuje o zasadach pielęgnacji tatuażu
• Nie ponoszę odpowiedzialności za nie przestrzeganie zasad pielęgnacji po zabiegu.
• Mam prawo odmówić wykonania tatuażu jeżeli zabieg zagraża zdrowiu lub życiu, lub kłóci się z moimi poglądami.
• Proszę o nie przyprowadzanie dzieci, lub kilku osób towarzyszących.
MAM PRAWO DO WYPROSZENIA OSOBY TOWARZYSZĄCEJ, JEŻELI ZAKŁÓCA ONA ZABIEG.
• Tatuaż wykonuje się na WŁASNĄ ODPOWIEDZIALNOŚĆ!
• W przypadku ZNACZĄCEGO(!) ubytku w tatużu po wygojeniu, przysługuje darmowa poprawa danego fragmentu (do 40 dni po wykonaniu tatuażu).
NIE PODLEGAJĄ POPRAWCE TATUAŻE WYKONANE NA SZYI, DŁONIACH, STOPACH, TWARZY, PALCACH.
• Z momentem wykonania tatuażu, klient wyraża zgodę na wykonanie zdjęcia lub filmu i opublikowanie go w portalach społecznościowych oraz na wykorzystanie go w celach marketingowych.
· ZABRONIONE JEST DOTYKANIE JAKICHKOLWIEK PRZEDMIOTÓW ZNAJDUJĄCYCH SIĘ NA STANOWISKU PRACY ARTYSTY!
· W przypadku przerwania zabiegu, nowy termin podlega oddzielnej opłacie.
W PRZYPADKU GDY OSOBA UMÓWIONA NA ZABIEG SPÓŹNI SIĘ 15 MINUT BEZ ŻADNEGO POWIADOMIENIA, LUB KONTAKTU , MAM PRAWO ODMÓWIĆ WYKONANIA ZABIEGU.
